M Beth Family Foundation LLC is an Adult and Child Developmental Homes Agency contracted with Arizona Department of Economic Security with Division of Developmental Disability. Our services include:
Our Providers together with DDD Personnel, Member’s family, and work on Members’ habilitation goals and how they can achieve them as they strive towards independence.
Our Members are supported and encouraged to engage in the community by going to school, working, and participating in other social activities in the community (If applicable) to be part of their community a hence sense of belonging. This also helps Members to excel in what they enjoy doing out in the community with supervision
The Agency Providers have individualized Care plans for our Members as per Member’s ISP (Individual Service Plan). Providers ensure that Member’s receive nutritional diet and work together with Member’s PCP and Service Coordinator to ensure Members Medical and dental appointments, Speech therapy, Occupational therapy, Behavioral therapies depending on the Member’s needs are attended to.
Member’s safety is Our Agency’s priority. The Agency licensing workers conduct thorough home study and inspection per licensing requirements to ensure that safety standards are in place before Members are placed in the homes.
The Providers also have continues/in-service training. This gives them confidence, a chance to improve, report on gaps, and keeping up with knew knowledge that will help best give services.
The Agency directors also conduct unannounced and announced to check and inspect the homes as per Arizona Department of Disability to ensure that the homes are inspected and continuously provide safe environment for our Members.
